GROUP INSURANCE IS WRITTEN TO PROVIDE COVERAGE TO DEFINED MEMBERS OF A GROUP.  WHEN GROUP INSURANCE IS WRITTEN, THE SPONSORING ORGANIZATION IS THE POLICYHOLDER AND EXERCISES CONTROL OVER THE PLAN.  A MASTER POLICY IS ISSUED TO THE SPONSORING ORGANIZATION WITH EACH INDIVIDUAL EMPLOYEE RECEIVING A CERTIFICATE OF INSURANCE AS EVIDENCE OF GROUP INSURANCE.  RATES WILL BE BASED ON THE GROUP AS A WHOLE, RATHER THAN INDIVIDUAL RATES.
